Thank you for your interest in helping us collect the elector signatures required for getting on the ballot for the August 11 primary and the November 3 general elections!

All signatures for federal candidate ballot nomination must be submitted to the Wisconsin Election Commission office by 5pm on 6/1/26.

- Both the circulator (the person asking collecting signatures) and signers must be 18 years of age or older and eligible to vote in the state of Wisconsin.
- Signers must reside in Wisconsin’s Congressional District 4 (map below, signers can also check https://myvote.wi.gov/en-us/My-Elected-Officials)
- Circulators and signers can only circulate or sign nomination papers for one candidate for each office, so please ask the signer if they have already signed the papers another Congressional District 4 candidate.
- As of 5/9/26, the following additional candidates are seeking ballot access and are circulating nomination papers per Ballotpedia: Carlos Dixon (Independent), Gwen Moore (Democrat, incumbent), Asher Smale (Democrat), Purnima Nath (Republican)
- As a circulator, you must witness the signature for it to be valid. This means you literally need to witness the signature as it happens, you cannot leave the page unattended.
- Signers should fill out the entire form, including Signatures of Elector, Printed Name of Elector, Residential Address, Municipality of Residence, and Date of Signing.
- Each person must indicate their house number & street name or rural route. PO Boxes are not acceptable addresses.
- The signer must use their address and municipality of residence, not their mailing address (if the two are different).
- Date of signing: the signer must use the date they are signing on. Please double check to make sure they used the correct date.
- As a circulator, once you’ve completed a sheet and/or are done collecting signatures, you must complete the “certification of circulator” on the bottom of the form saying you personally obtained each signature.
- Write your name, address of residence, signature, and date.
- You (the circulator) should only fill the bottom of the sheet out once you’ve collected all the signatures you plan to collect for that page.
- The page does not need to be filled, but again, do not collect any more signatures on a page you’ve already certified.
- You may sign and certify your own signature! Every signature helps, even if you don’t want to collect other signatures, you can sign yourself, certify, and get it back to Amy’s campaign.
- Please do not number the pages on the bottom of the form. The campaign will number the pages before we submit them.
- Please do not submit a copy or scan of the sheets, the campaign can only accept original sheets! When you are ready to return your sheet(s), contact campaign@amydonahueforcongress.com or 414.261.3120 to let us know how many signatures you have and whether you would like to:
- Connect with the campaign for a physical handoff (we can pick up or tell you where to drop off)
- Mail in your sheet(s). Mailed sheets should be sent ASAP, and ideally no later than May 21st.

- A qualified elector of Wisconsin is someone who:
- Is a U.S. citizen
- Is at least 18 years old
- Has resided at their residential address in Wisconsin for at least 28 consecutive days
with no present intent to move - Is not currently serving a felony conviction, including incarceration, parole, probation,
or extended supervision - Has not been adjudicated to be incompetent with a finding that they do not understand
the voting process - Has not made a bet or wager on the result of the election
